Project Title: Community-Led Integrated Health Promotion and Referral Support in Zango LGA, Katsina State

1. Background
RAHAMA Girl Child and Women Empowerment Initiative, in partnership with Médecins du Monde (MdM), is implementing a project titled “Community-Led Integrated Health Promotion and Referral Support” in Zango LGA, Katsina State. The project aims to improve public health outcomes and health system responsiveness by enhancing health literacy, promoting access to services, strengthening community structures, and establishing effective referral and feedback mechanisms. As part of the intervention, RAHAMA is recruiting qualified and passionate individuals to serve as Field Officers and Case Management Officers (CMOs) to support community-level implementation and GBV response.
2. Purpose of the Position
Both positions are essential to advancing the project’s objectives at the grassroots level. Field Officers will lead community sensitization, active disease surveillance, follow-up of identified cases, and support immunization efforts. Case Management Officers will provide survivor-centered support to individuals affected by gender-based violence (GBV), strengthen referral systems, and facilitate safe spaces for communities.
3. Key Responsibilities
Successful candidates will be responsible for:
- Conducting weekly community sensitizations and outreach to promote health, protection, and service uptake.
- Identifying and reporting suspected disease cases, and supporting referrals in collaboration with health officials and community informants.
- Supporting the coordination and delivery of routine immunization and public health campaigns.
- Providing comprehensive support to GBV survivors, including case intake, referral, follow-up, and coordination with service providers.
- Working closely with community structures, men's women’s groups, and youth networks to strengthen grassroots health and protection systems.
- Participating in LGA-level coordination meetings and supporting data collection and documentation through digital tools such as KoboCollect.
- Upholding confidentiality, professionalism, and safeguarding principles at all times.
